Cancellation Policy
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24-hours notice. Failure to do so will result in a $50 no-show/late cancellation fee for office visits and a $100 no-show/late cancellation fee for surgery. This courtesy on your part will make it possible for other patients to fill available appointment times.

We accept most traditional insurance plans. Please contact our office to verify acceptance of your plan. Qualifications for insurance coverage may differ due to the uniqueness of each procedure.
